¿Existe relación entre la calidad de vida y las morbilidades postmastectomía en mujeres diagnosticadas con cáncer de mama?

A nivel mundial el cáncer de mama es el primero en frecuencia en mujeres. En
Chile es la primera causa de muerte y su incidencia estimada por el Programa
Nacional de Cáncer de Mama fue cercana a los 100.000 casos nuevos el1_909.
Se ha evidenciado que existen numerosas complicaciones luego de una
mastectomía ya sea parcial o total, con ganglio centinela y/o vaciamiento axilar.
Estas complicaciones podrían tener algún imp cto sobre la Calidad de Vida de
las personas con cáncer de mama, por lo que se planteó realizar una
investigación que las describiera y analizara por si solas, para así poder
relacionarlas con la calidad de vida.
Con éste fin se optó por llevar a cabo una investigación, de tipo\...a_sociativo
observacional de corte transversal con énfasis en la descripción. Se evaluó a 23
mujeres con diagnóstico médico de cáncer de mama que fueron intervenidas
quirúrgicamente en el Hospital San Martín de Quillota y Hospital de Quilpué y
que se encuentren entre la cuarta y sexta semana post cirugía. Se realizó una
serie de evaluaciones para evidenciar posibles complicaciones, tales como:
aumento de volumen, alteraciones musculoesqueléticas, síndrome de red axilar
y dolor, y así, evidenciar el impacto de éstas en la Calidad de Vida de las
mujeres a través del cuestionario EQ5D.
Lo resultados arrojaron información importante al relacionar las complicaciones
mJncionadas anteriormente y la Calidad de Vida de cada una de las personas
evaluadas. En cuanto a los acortamientos musculares y la disminución de los
rangos de movimiento, se pudo comprobar que existe una muy fuerte relación
entre ambos, con respecto a la prueba de funcionalidad del test de Apley,
queda en evidencia que uno de los rangos que comúnmente resulta limitado es
la keparación. Estas complicaciones, sumado al dolor que presenta la persona
enj distintas zonas del cuarto superior, conlleva al posicionamiento de la
extremidad que favorece los trastornos mencionados anteriormente.
c9n éstos resultados se llegó a la conclusión que debido al alto número de
morbilidades asociadas a la intervención quirúrgica postmastectomía, se cree
que es necesario proveer el acceso a una intervención kinésica adecuada y
opbrtuna, con el fin de evitar las complicaciones y trastornos mencionados en
este estudio.
Globally, Breast Cáncer is woman's first frecuency. In Chile, is the first cause of death and it is incidence estimated by the Nacional Breast of Cancer Program was close to 100.000 new cases in 2009. lt has been shown that there are numerous complications after Mastectomy either Partial or Total with Sentinel Node and/or Axillary Lymphadenectomy. This complications could have an impact in Breast Cancer woman's quality of life. So it was decided to perform an investigation that describe and analyze it for themselves, in order to reate quality of life. Knowing this complications, it could be performed a kinesics post operative intervention to prevent or reduce the incidence of this. For this end, it was decided to carry on an investigation, of associative observational tipe, transversal incicive with emphasis on the description. We evaluated 23 women with a medica! diagnosis of Breast Cancer who underwent surgery at the Hospital San Martín of Quillota and Quilpue's Hospital situated between the fourth and sixth week post surgery. A serie of severa! evaluations to revea! possible kinesics complications it was made, there were: volume increase, musculoskeletal disorders, axillary web syndrome and pain, and thus, demonstrate their impact on woman's quality of life through EQ5D questionnaire. The result yielded important information by relating the complications before mentioned and quality of life of each people tested. In terms of muscle shortening and decreased range of motion, it was found that there is a very strong relationship between them. Regarding functionality Apley test, it was clear that one of the ranges that commonly results limited is separation. According to EV evaluation with quality of life, significantly, it's inversely proportional, because the more pain, decreased quality of life, this, makes a lot of sense with the literature above, therefore, people who have un6ergone Mastectomy either total or partial, with axillary lymphadenectomy and/or sentinel node, have pain. This complications, added to the individual pain present in severa! areas of the upper limb related by the person, leads to the lirrib positioning, favoring to the aforementioned disorders. With this results, it concluded that due to the high number of morbidities associated with the Mastectomy surgery, is necessary to provide access to an appropriate timely kinesics intervention, in order to avoid the complications and disorders mentioned in this study.
