Objective: To determine the prevalence and demographic characteristics of the middle mesial canal (MMC) in lower molars of the V region of Chile, identifying its anatomical configuration, direction and end of its trajectory by means of cone beam computed tomography. Materials and methods: 844 molars (422 first and 422 second molars) taken from 211 scans (83 men and 128 women between 12 and 50 years old) were analyzed. By means of a multiplanar tomographic analysis with 0.2 mm slices, the presence of MMC was established, classifying it according to its anatomical configuration, direction and end of the path. The data were statistically analyzed with a 95% confidence interval. Results: Of the total of molars analyzed, 24 presented the MMC (2.84%) being, of the total of these, 95.83% (n = 23) first and 4.17% (n = 1) second molars. Its most prevalent configuration was confluent (n = 17), 14 of them ending in the middle third of the root in the direction of the mesio-vestibular canal (n = 10). A significantly higher prevalence was observed in men, but not with respect to the age range. Conclusion: The prevalence of MMC in the studied population is low, however, these agree with the ranges reported in the literature, with a significant predilection in men and not in age. The most frequent configuration was the confluent one, ending, in most cases, in the middle third.
