Resumen
La ModulaciĆ³n Condicionada del Dolor (CPM) puede ser modificada por experiencias o factores externos y difiere segĆŗn inidividuos. Objetivo: Evaluar y comparar la modulaciĆ³n condicionada del dolor en mujeres de alto rendimiento atlĆ©tico y mujeres no atletas con dolor miofascial. MĆ©todo: Se examinaron a 10 mujeres con dolor muscular y sin dolor articular (RDC/TMD) y 13 mujeres atletas de alto rendimiento en un estudio de caso-control. Se midieron los mĆŗsculos maseteros, temporales y braquialis del brazo en una ocasiĆ³n por algometrĆ­a para obtener umbral de dolor a la presiĆ³n (PPT) y tolerancia al dolor (PPTol). CPM se realizĆ³ mediante nuevas medidas de algometrĆ­a en los mismos mĆŗsculos, pero bajo un estimulo de frio en la mano (baƱo de inmersiĆ³n 6ĀŗC). Se analizĆ³ afectos positivos y negativos, recompensa y castigo, felicidad, catastrofismo, regulaciĆ³n emocional y apoyo, donde los datos se analizaron por chi2 , Fischer exacta , U de Mann - Withney , Kruskal Wallis y el test T. Resultados:Los anĆ”lisis de PPT Muscular fueron similares para atletas y para pacientes; el temporal izquierdo fue el mĆŗsculo con menor umbral (P=0,02); PPTol fue mayor para atletas en diferentes mĆŗsculos (P<.05). CPM fue similar entre los grupos. (P>.05). La mayorĆ­a de las mujeres fueron respondedoras al dolor y tanto atletas como pacientes tenĆ­an habilidades moduladoras similares. Las atletas presentaron en general mayores puntuaciones en las dimensiones psicomĆ©tricas positivas y menores valores en las dimensiones negativas, excepto en catastrofismo, rumiaciĆ³n y magnificaciĆ³n. Conclusion: Mujeres Atletas y con dolor orofacial presentan habilidades de modulaciĆ³n condicionada de dolor similar. Las atletas fueron mĆ”s tolerantes al dolor a la presiĆ³n y tienen diferencias en variables psicolĆ³gicas.
Abstract: Conditioned pain modulation (CPM) can be modified by external activities or experiences and differ among individuals Objective: To evaluate and compare conditioned pain modulation in healthy high performance female athletes to non-athletes women with orofacial myofascial pain. Method: A Case- Control Study was performed. 10 women athletes without muscular or joint pain (RDC / TMD) and 13 highperformance female athletes were compared. Masseter, temporal and braquialis muscles were measured by algometry to asses pressure pain threshold (PPT) and pain tolerance (PPTol). CPM was performed by new algometry measures in the same muscles afterhand immersion in a cold bath (6ĀŗC water as conditioned stimuli). Positive and negative affects, reward and punishment, happiness, catastrophizing, emotional regulation and social support where analyzed as pscimetric variables. Data were analyzed by chi2, Fischer exact, U-Mann Withney, Kruskal Wallis and T test in SPSS v22. Results: Muscular PPT analysis where similar for athletes and patients and Left Temporalis was the only muscle with lower threshold (P=0,02), PPTol was higher in athletes muscles (P<.05). Most women where pain responders and CPM was similar between them (P>.05). Athletes presented better positive affects and lower negative psychometric dimensions but presented more catastrophizing, rumination and magnification. Conclusion: Women with Orofacial pain and highly trained athletes have similar Conditioned Pain Modulation Abilities. Woman athletes were more tolerant to pain pressure and present psicological differences.
