Antecedentes: Las rugas palatinas son elevaciones de la mucosa, van en
numero de dos a seis y se extienden en sentido transversal desde la papila
incisiva hacia la periferia. Dentro de la rehabilitación protésica maxilar removible
juegan un rol fonoarticulatorio poco estudiado. No existen estudios nacionales
acerca de este tema, por lo que la importancia fonoarticulatoria de las rugas
palatinas en la rehabilitación protésica removible maxilar, sigue siendo una
interrogante.
Justificación: Evaluar la importancia fonoarticulatoria de las rugas palatinas en
la rehabilitación protésica del paciente desdentado total y parcial, se
fundamenta en la necesidad de otorgar una condición fonoarticulatoria
favorable, logrando así el éxito de la rehabilitación.
Objetivo: Determinar la importancia fonoarticulatoria de las rugas palatinas
dentro de la rehabilitación protésica removible maxilar, en los pacientes
portadores de prótesis total maxilar.
Material y Métodos: Investigación de tipo cuantitativa, analítica, experimental,
comparativa, en pacientes atendidos en la Clínica de la Universidad Andrés
Bello. Se seleccionó una muestra no probabilística, intencionada, de pacientes
pertenecientes a 3 grupos: dentados naturales, portadores de prótesis maxilar
removible sin rugas y con rugas palatinas. Para efectos de comparación
fonoarticulatoria se intervino uno de los grupos desdentados, incorporándole
rugas palatinas a su prótesis. A los cuales se les aplicó un test articulatorio a la
repetición, los resultados fonoarticulatorios fueron analizados mediante un
programa de análisis y reconstrucción de señales de voz acústicas (PRAAT
versión 5.4.08), para el análisis estadístico, se utilizo el software SPSS versión
22. Se evalúo normalidad en la distribución de las variables y de acuerdo a los
resultados observados, se aplicó el test de Kruskall-Wallis, que mide asociación
entre dos o más grupos.
Resultados: Las rugas palatinas demostraron tener un rol dentro de la
producción de los fonemas. Variando las características acústicas, donde
existió diferencia en los fonemas alveolares, siendo estadísticamente
significativas para los fonemas N, L, R y RR. Los pacientes cuyas prótesis
tenían rugas palatinas personalizadas en sus bases, produjeron fonemas
acústicamente muy similares a aquellos pacientes que eran dentados
naturales, a diferencia de los que no las poseían. Esta situación reviste real
importancia dentro de la rehabilitación integral del paciente.
Conclusión: Los pacientes cuyas prótesis removibles maxilares tenían rugas
palatinas incorporadas, presentaron mejores condiciones fonoarticulatorias
para los fonemas evaluados en esta investigación, en comparación los
pacientes que no las poseían. La confección de rugas palatinas en la base
protésica mejoró el punto de articulación de los fonemas estudiados en esta
investigación.
Background: The palatal rugae correspond to elevations of the mucosa in number from two to six extending transversely from the incisive papilla to the periphery. Within maxillary removable prosthetic rehabilitation plays a phonetic role still poorly studied. There isn’t national studies on this subject, so the phonetic importance of palatal rugae in maxillary removable prosthetic rehabilitation, remains a question. Justification: Evaluate the phonetic importance on speech of palatal rugae in prosthetic rehabilitation of the total and partial edentulous patients, it’s based on the need to provide a favorable condition of speech, thus ensuring the success of rehabilitation. Purpose: Determine the phonetic importance on speech of palatal rugae within the maxillary removable prosthetic rehabilitation. Material and Methods: The research methodology is a quantitative, analytical, experimental, comparative, in patients treated at the Dental Clinic of the University Andres Bello. The sample consisted of 30 patients, belonging to 3 groups: with natural teeth, carriers of dental prosthesis without incorporated rugae and the last group with rugae incorporated dentures, made in the Dental Clinic of the University Andres Bello between 2014 and 2015. We applied an articulatory repetition test (TAR), articulatory test results were analyzed by a program of analysis and reconstruction of acoustic voice signals (PRAAT version 5.4.08). Then the test results were compared by applying statistical tests to evaluate statistically significant differences in fonoarticulación as comparison groups. Results: The palatal rugae shown to have a role in the production of phonemes. By varying the acoustic characteristics, which existed in the alveolar phonemes difference being statistically significant for the phonemes N, L, R and RR. Patients whose prosthesis had palatal rugae custom built into their bases, produced acoustically similar phonemes patients who were natural jagged, unlike those who did not. This situation is of real importance in the integral rehabilitation. Conclusion: Patients whose dentures with palatal rugae are incorporated, had better conditions of speech, for the phonemes evaluated in this study, compared patients who did not have them. The preparation of palatal rugae in the denture base improved articulation point of phonemes studied in this research
