ANTECEDENTES
Como tratamiento de la Estomatitis subprotésica se han utilizado una serie de terapias, entre ellas destaca la aplicación de gel de Aloe vera. En el año 2007, se realizó el estudio “Eficacia de la crema de Aloe al 25% en la Estomatitis Subprotésica grado II”, teniendo como resultado una curación más rápida en comparación a aquellos pacientes que pertenecían al grupo control, sumado a la indicación de retiro protésico nocturno.
Sin embargo, las investigaciones sobre Aloe vera en el área odontológica siguen siendo escasas, por lo cual surge la necesidad de efectuar nuevas investigaciones en este campo.
JUSTIFICACIÓN
Esta investigación se fundamenta en la necesidad de evaluar el poder antiinflamatorio como tratamiento a la Estomatitis subprotésica y así entregar un tratamiento alternativo en el área odontológica. OBJETIVO
Determinar la efectividad de la aplicación de gel de aloe vera al 25% en el tratamiento de estomatitis subprotésica tipo I, II y III.
MATERIAL Y METODOS
Estudio de campo con abordaje cuantitativo cuyo diseño es experimental: ensayo clínico controlado, intersujeto en pacientes portadores de prótesis removibles parciales y totales pertenecientes a clínica odontológica UNAB. La muestra no probabilistica, intencionada, estuvo constituida por 14 pacientes que cumplieron criterios de inclusión y que consintieron informadamente a participar en el estudio.
El grupo control recibió el tratamiento convencional que consistió en el desgaste de 1 mm de acrílico en la parte interna de la prótesis, logrando una superficie libre de rugosidades, para su posterior aplicación de acondicionador de tejido; se le indicó al paciente mantener una adecuada higiene oral, retiro nocturno de la prótesis y masajear zona del paladar con lengua para fomentar la irrigación. El grupo experimental recibió el tratamiento propuesto en esta investigación que consiste en el tratamiento con crema de Aloe vera al 25%, aplicada en capa fina sobre la superficie afectada 3 veces al día, después de la ingesta de comida, manteniendo una adecuada higiene, y se les orientó no ingerir agua ni otro alimento durante una hora después de aplicado el tratamiento y realizar la última aplicación antes de dormir y retirada inmediata de la prótesis siguiendo con el masajeo de la zona afectada. La evaluación de la efectividad de la técnica de tratamiento fue evaluada por juicio de experto, considerando remisión a los 7 y 8 días de tratamiento. Los resultados obtenidos fueron procesados con software SPSS versión 24, utilizando para el análisis de prevalencia medidas descriptivas y test de tipo no paramétrico para evaluar asociación entre días de remisión de la patología y tratamiento recibido (Chi- cuadrado y V de Cramer).
RESULTADOS OBSERVADOS
La prevalencia de Estomatitis subprotésica se observó en el 100% de los pacientes evaluados, cuyo promedio de edad fue de 65,07 años. El género femenino fue el más afectado y el tipo más frecuente de Estomatitis correspondió al tipo I con un 64,3%.La efectividad del tratamiento de Estomatitis subprótesica con el tratamiento de gel de Aloe vera al 25%, fue de 71,4% con remisión a los 7 días y de 28,6% a los 8 días; en comparación con el tratamiento de acondicionador de tejidos, donde el 51,7% remitió a los 7 días y el 42,9% a los 8 días. Por ende, no se observó diferencias estadisticamente significativos en los tiempos de remisión de ambos tratamientos pero si se demuestra que tienen la misma efectividad para el tratamiento tópico de Estomatitis subprotésica.
CONCLUSIONES
La mayor prevalencia de Estomatitis subprotésica se observó en pacientes de sexo femenino mayores a 65 años.
Para el tratamiento de Estomatitis subprotésica, no existió diferencia signiificativa entre los días de remisión entre el preparado de Aloe vera y el tratamiento con acondicionador de tejido, por lo cual se concluye que es eficaz en el tratamiento de esta patología y que puede ser utilizado de manera alternativa.
BACKGROUND As treatment of denture stomatitis have been used a number of therapies, among them the application of Aloe vera gel. In 2007, the study "Effectiveness cream Aloe 25% in denture stomatitis grade II" was performed, resulting in faster healing compared to those patients who were in the control group, together with the indication night prosthetic retreat. However, research on Aloe vera in the dental area is still scarce, so the need for further research in this field arises. JUSTIFICATION This research is based on the need to evaluate the anti-inflammatory power as a treatment for sub-prosthetic stomatitis and to provide an alternative treatment. PURPOSE Determine the effectiveness of the application of aloe vera gel 25% in the treatment of denture stomatitis type I, II and III. MATERIALS AND METHODS Field study with quantitative approach whose design is experimental: controlled clinical trial, intersubject in patients with removable prostheses and totals belonging to the dental clinic UNAB. The non-probabilistic consisted of 14 patients who met inclusion criteria and who informed consent to participate in the study. The control group received the conventional treatment consisting of the wear of 1 mm of acrylic in the inner part of the prosthesis, achieving a surface free of roughness, for its subsequent application of fabric conditioner; the patient was instructed to maintain adequate oral hygiene, nocturnal removal of the prosthesis and massage of the palate with tongue to encourage irrigation. The experimental group received the treatment proposed in this investigation consisting of the treatment with crema Aloe vera 25%, applied in thin layer on the affected surface 3 times a day, after the food intake, maintaining adequate hygiene, and They were advised not to ingest water or other food for one hour after the treatment was applied and to perform the last application before bed and immediate removal of the prosthesis following the massage of the affected area. The evaluation of the effectiveness of the treatment technique was evaluated by expert judgment. The results were processed using SPSS software version 24, using descriptive measures for prevalence evaluation and non-parametric type tests to evaluate the association between days of remission of the disease and treatment received (Chi- cuadrado and V of Cramer) . EXPECTED OUTCOMES The prevalence of denture stomatitis was 100%, where the mean age affected was 65.07 years. The female gender was the most affected and the most frequent type of stomatitis corresponded to type I with 64.3%. The effectiveness of the denture stomatitis treatment with Aloe vera gel treatment at 25% was 71.4% at 7 days and 28.6% at 8 days; Compared to the treatment of tissue conditioners, where 51.7% relapsed at 7 days and 42.9% at 8 days. Thus, both treatments are equally effective for topical treatment of denture stomatitis. CONCLUSIONS The highest prevalence of sub-prosthetic stomatitis was observed in female patients older than 65 years. For the treatment of subprotésic stomatitis, there was no significant difference between the days of remission between the preparation of Aloe vera and the treatment with tissue conditioner, for which it is concluded that it is effective in the treatment of this pathology and that can be used of Alternative way.
