Aplicación de un protocolo de liberación miofascial en pacientes con diagnóstico de enfermedad del manguito rotador y alteraciones de la funcionalidad de hombro, pertenecientes al nivel primario de atención

La enfermedad del manguito retador presenta una prevalencia de un
5,2% en centros de atención primaria de salud (Holmes et al ,2013) y tiene una
significativa repercusión en el ámbito laboral y social de los individuos que la
padecen.
Existen estudios que apoyan y recomiendan de manera aislada
algunas herramientas de kinesiterapia tradicional, pero hay una falta de
evidencia en la actualidad con respecto a su uso en la práctica clínica (Panel
de Philadelphia, 2000). El interés surgido por el sistema facial en los últimos
tiempos, como estructura unificadora, sumado a la escasez de estudios sobre
esta temática en el medio nacional que se orienten a la funcionalidad de
hombro en pacientes diagnosticados con enfermedad del manguito retador,
ha sido el motor para que los encargados de esta investigación, elaboren un
protocolo de terapia miofascial, basado en la aplicación de técnicas de
liberación miofascial superficial y profunda.
El objetivo principal de este estudio ha sido evaluar la
funcionalidad de hombro en sujetos con diagnóstico de enfermedad del
manguito retador a través de una pre y post prueba introduciendo el manejo
del tejido miofascial. Para tal efecto se trabajó con 30 sujetos con edades
promedio de 50,5 años, distribuidos en 2 grupos, uno experimental y un grupo
control y se midió el estado funcional de la articulación del hombro a través del
test de UCLA El grupo control fue intervenido con tratamiento kinésico convencional y el grupo experimental, recibió, además el protocolo de
liberación miofascial. Para el análisis estadístico se utilizó el test de Wilcoxon
y la t de student. Los principales resultados han revelado que ambos
grupos, experimentaron un cambio estadísticamente significativo, sin embargo
es muy importante señalar que al comparar las variaciones de los resultados
de ambos grupos, la mediana del grupo experimental correspondió a un valor
de 13 y la del grupo control a un valor de 11, cambios no estadísticamente
significativos.
The rotator cuff disease has a prevalence of 5.2% in primary health care (Holmes et al, 2013) and has a significant impact on the labor and social sphere of the individuals who have it. There are studies that support and recommend sorne tools in isolation traditional physiotherapy, but there is a lack of evidence today regarding its use in clinical practice (Philadelphia Panel, 2000). The interest by the facial system emerged in recent times, as a unifying structure, coupled with the scarcity of studies on this subject in the national media that are geared to the functionality of shoulder in patients diagnosed with rotator cuff disease, has been the engine for managers of the research , to develop a protocol myofascial therapy, based on the application of techniques of superficial and deep myofascial release. The main objective of this study was to evaluate the functionality shoulder in subjects diagnosed with rotator cuff disease through a pre and post test introducing management myofascial tissue. To this end, we worked with 30 subjects with an average age of 50.5 years, divided into 2 groups, one experimental and one control group and the functional state of the shoulder joint through the test of UCLA was measured. The control group underwent conventional physiotherapy treatment and the experimental group also received the protocol myofascial release. For statistical analysis the Wilcoxon test and the Student t test was used. The main results revealed that both groups experienced a statistically significant change, however it is very important to note that when comparing changes in the results of both groups, the median experimental group corresponded to a value of 13 and the control group a value of 11 , no statistically significant changes
