Evaluación del programa de rehabilitación vestibular, a través de la encuesta DHI, en una serie clínica de pacientes con patología vestibular periférica

Resumen
El Equilibrio se logra por la integración principalmente de los sistemas
vestibular, propioceptivo y visual en el SNC, permitiendo mantener la postura, caminar
y realizar eficazmente las actividades diarias. Cuando las señales vestibulares entran en
contradicción con las procedentes de otros sistemas aferentes, surge la sensación
vertiginosa, generando que los pacientes pierdan la capacidad para desarrollar la
mayoría de sus actividades diarias en forma normal. Una vez desencadenado el daño
vestibular, el organismo da paso a la compensación vestibular. La aplicación del
programa de ejercicios de Rehabilitación Vestibular en pacientes que padecen de
patologías periféricas del Sistema Vestibular, tiene como objetivo generar nuevas redes
de conexiones neurales, cuyo fin es suplir la falla del aparato vestibular y así potenciar
los otros dos sistemas, que actúan en el equilibrio, permitiendo al paciente desarrollar
sus actividades cotidianas normalmente. La encuesta DHI se ha diseñado para evaluar el
impacto del vértigo en la vida diaria, en este estudio es utilizada para evaluar la
efectividad de la rehabilitación vestibular en pacientes con patología vestibular
periférica. Método: Estudio experimental explicativo, en el periodo de Agosto a
Noviembre del 2008, en el servicio de Otorrinolaringología del Hospital Clínico de la
Universidad Católica. Fueron incluidos 35 pacientes con patología vestibular periférica,
se les aplico el programa de rehabilitación vestibular y se midió la eficacia de este a
través de la encuesta DHI, que permite evaluar el estado físico, funcional y emocional
del paciente. Resultados: Los resultados indican una completa relación entre el aspecto
físico y el aspecto emocional – funcional, tanto en el daño como en su mejoría.
Conclusión: La rehabilitación vestibular es efectiva, logrando que la mayoría de los
pacientes alcance una discapacidad leve, una de las patologías que más de beneficia con
el programa de rehabilitación vestibular es la paresia vestibular unilateral, alcanzando su
mayor porcentaje de mejoría en E1 E2.
The Balance is achieved by the integration principally of the systems vestibular, propioceptive and visually in the SNC, allowing to support the position (attitude), to walk and to realize effectively the daily activities. When the vestibular signs enter contradiction with the proceeding ones from other systems afferents, the dizzy sensation arises, generating that the patients lose the aptitude to develop the majority of his(her, your) daily activities in normal form. Once unleashed the vestibular hurt (damage), the organism (organization) gives step to the vestibular compensation. The application of the program of exercises (fiscal years) of Vestibular Rehabilitation in patients who suffer of pathologies peripheral from the Vestibular System, has as aim (lens) generate new networks (nets) of neural connections, which end (purpose) is to replace the fault of the vestibular and like that device to promote other two systems, which act in the balance, allowing to the patient to develop his (her, your) daily activities normally. The survey DHI has been designed to evaluate the impact of the dizziness in the daily life, in this study it is used to evaluate the efficiency of the vestibular rehabilitation in patients with vestibular peripheral pathology. Method: Experimental explanatory study, in the period from August to November, 2008, in the service of Otolaryngology of the Clinical Hospital of the Catholic University. 35 patients were included by vestibular peripheral pathology, I apply to them the program of vestibular rehabilitation and the efficiency measured up of this one across the survey DHI, which allows evaluating the physical, functional and emotional condition (state) of the patient. Results: The results indicate a complete relation between (among) the physical aspect and the emotional aspect - functionally, both in the hurt (damage) and in his (her, your) improvement. Conclusion: The vestibular rehabilitation is effective, achieving that the majority of the patients reaches a slight disability, one of the pathologies who more of is of benefit with the program of vestibular rehabilitation is the vestibular unilateral paresis, reaching his (her, your) major percentage of improvement in E1 E2.
